Dental Waste Disposal

Dental practices produce: infectious clinical waste (dressings, swabs, and consumables contaminated with blood or body fluids); sharps waste (needles, scalpels, burs); amalgam waste (mercury-containing); pharmaceutical waste (expired or unwanted medicines); hazardous chemical waste (disinfectants, radiographic chemicals where X-ray is used); and general domestic waste. Each stream requires appropriate segregation and disposal.

Amalgam waste should be stored in sealed, clearly labelled amalgam containers provided by the hazardous waste contractor. Keep containers in a secure, ventilated area away from heat sources, mercury vapour release is a concern if containers are damaged or stored in warm conditions. Do not mix amalgam waste with other clinical or domestic waste. Maintain waste segregation even when containers are awaiting collection.

Extracted teeth containing amalgam restorations are classed as amalgam-containing hazardous waste and must be disposed of separately from standard clinical waste. They should be stored in amalgam containers alongside other amalgam waste and collected by a licensed hazardous waste contractor. Extracted teeth without amalgam restorations are clinical waste and can be disposed of in yellow clinical waste bags.

Hazardous waste producers must complete consignment notes for each collection of hazardous waste. For dental amalgam waste, keep signed consignment notes from each collection for a minimum of 3 years. The CQC may request evidence of compliant waste management during inspections. Ensure your waste contractor is licensed to transport and dispose of hazardous waste and provides consignment documentation as part of their service.

Sharps bins have a fill line indicator, usually marked in yellow at approximately two-thirds of the container capacity. Do not fill above the fill line, overfilling increases the risk of sharps protruding through the closure mechanism and causing needle-stick injuries during handling and transport. Seal the bin at the fill line, complete the label with practice details and date of closure, and set aside for collection.

X-ray fixer and developer chemicals are hazardous chemical waste and must be disposed of separately from clinical waste bags. They require disposal as chemical hazardous waste through a licensed contractor. Many practices have moved to digital X-ray systems which eliminate this waste stream. If you still use film-based radiography, ensure your waste contractor is licensed for chemical hazardous waste collection.

The frequency depends on practice volume and container sizes. A busy multi-surgery dental practice may require weekly clinical waste collection and monthly amalgam and sharps collection. A smaller single-surgery practice may manage with fortnightly clinical waste and quarterly sharps and amalgam. Arrange collection frequency based on how quickly containers reach capacity, never allow containers to overflow or be overfilled before collection.

Dental hazardous waste must be collected by a carrier registered with the Environment Agency to transport hazardous waste. Check that your waste contractor holds a valid Environment Agency carrier registration covering the waste types you generate (clinical, amalgam, pharmaceutical). Request copies of their registration and insurance. Using an unlicensed carrier is an offence for the waste producer as well as the carrier.
