The disposal of controlled drugs in the UK is governed by the Misuse of Drugs Regulations 2001 and associated guidance. A controlled drug disposal kit, also known as a denaturing kit, contains a denaturing agent that renders controlled drugs irretrievable when mixed together in the kit's sealed container. The drug and denaturing agent are combined in the container, which is then sealed and disposed of as pharmaceutical or clinical waste. Kits are available in various sizes and come with full instructions for compliant use. Using a validated kit provides an auditable, witnessed destruction method.

Drug disposal kits are suitable for liquid and tablet-form controlled drugs including morphine solutions, methadone, other opioids, and benzodiazepines. They are not appropriate for sharps disposal, needles and syringes must be disposed of separately in an approved sharps container. Non-controlled prescription medicines and over-the-counter medicines that are no longer required can typically be returned to a community pharmacy for safe disposal.

In care homes in England, Wales, and Scotland, the destruction of schedule 2 controlled drugs must be witnessed by an authorised person. Since 2013, this can include a pharmacist, a registered nurse in certain roles, or a local supervising pharmacist depending on the jurisdiction. A written record of each destruction must be maintained. The care home manager should review current guidance from the CQC, Care Inspectorate Wales, or equivalent regulator as requirements can vary.

A controlled drug register must be maintained for schedule 2 drugs, recording the date of receipt, quantity received, amount administered, amount destroyed, balance in stock, and the signature of the witness to destruction. Records must be kept for a minimum of two years. The register should be a bound book with sequentially numbered pages, a loose-leaf or computer-based system is not acceptable as the primary register. Regular audits of the register against stock should be carried out and any discrepancies investigated immediately.

Medicines returned by patients or members of the public to a community pharmacy are collected free of charge and disposed of through a patient returns scheme. Non-controlled medicines are collected in sealed waste containers and disposed of by a licensed pharmaceutical waste contractor. Controlled drugs returned by patients are recorded and destroyed in accordance with the relevant regulations. Pharmacies cannot sell or reuse returned medicines, all returns must be treated as waste regardless of their condition.

Controlled drug disposal kits can be used for expired controlled drugs that are no longer fit for use. Expired non-controlled medicines should be returned to a community pharmacy or collected by a licensed pharmaceutical waste contractor, they must not be placed in general waste or flushed. Expired stock in care homes and clinical settings should be reviewed regularly and disposed of through the appropriate waste stream, with records maintained as required by the relevant drug handling regulations.

Commercial drug disposal kits are validated for compliant destruction of controlled drugs and include the denaturing agent necessary to render drugs irretrievable, a requirement of the Misuse of Drugs Regulations. Standard waste bins or bags cannot be used for controlled drug disposal and are not a legal alternative. Commercial kits are supplied with full instructions, compliance documentation, and a sealed, tamper-evident container that provides an auditable record of destruction. Using a compliant kit protects the facility from regulatory risk.
